Our commitment to excellence is reflected in our comprehensive rewards program, which we recognize is a significant part of our employees' lives. The program is designed to attract and retain the very best talent the Life Sciences industry has to offer. We review both compensation and benefit plan design annually to ensure we meet and exceed the competitive industry standards that currently exist.

The compensation portion of the rewards package comprises a competitive base salary, short-term cash (bonus) and long-term equity incentives (stock options). Our success is based on the outstanding performance of our team members and of the Company. We strive to reward that performance regularly.

We know that health benefits are essential to our employees and their families. Covered dependents include spouse, domestic partner and eligible children to age 26. Sunesis offers the following plans for all employees working at least 30 hours per week:

Medical Insurance — Sunesis offers a choice of HMO or PPO coverage so you may elect the option that best aligns with your health care needs. Both options include a prescription drug benefit. The employee premium contribution is minimal and is processed on a pre-tax basis.

Dental Insurance — Our Dental PPO Plan allows you to select your favorite Dentist. Sunesis pays 100% of the premium for employees and eligible dependents.

Vision Plan — Offered through VSP, coverage provides an exam and lenses once every 12 months, frames every 24 months. Discounts are available at in-network providers. Sunesis pays 100% of the premium for employees and eligible dependents.

401(k) Plan — The Sunesis Retirement Savings Plan is administered by Fidelity and offers both traditional and Roth 401(k) options. With a diverse selection of investment funds to choose from, employees may contribute 1% to 60% of cash compensation (pre-tax), up to the annual IRS limit. Subject to annual approval, Sunesis provides an employer match at year-end, dollar for dollar up to $2,500 annually, that is 100% vested once it hits your account.

Flexible Spending Accounts — Employees may save money on a pre-tax basis for eligible out-of-pocket health care and dependent care expenses.

Employee Stock Purchase Plan — The ESPP allows employees to purchase Sunesis Common Stock through payroll deduction at a discount.

Life & AD&D Insurance — Your life insurance benefit is equal to two times base salary up to $500,000. The AD&D benefit is equal to another two times your base salary up to $500,000. Sunesis pays 100% of the premium for both these benefits.

Short-term Disability — This Sunesis benefit pays 100% of your base monthly salary, up to 90 days, and is coordinated with any applicable State Disability Insurance.

Long-term Disability Insurance — The long-term disability insurance provides a salary-continuation benefit equal to 60% of your monthly base salary up to $10,000 per month. Your benefit may continue through normal retirement age with continued disability certification. Sunesis pays 100% of the cost of every employee's long-term disability premium.

Employee Assistance Program. When personal challenges arise, the EAP is available to employees and their eligible dependents to provide confidential assistance. Consultations can be arranged for child or eldercare assistance, relationship or family problems, financial stress, depression, anxiety, bereavement or grief counseling.

Paid Time Off — Sunesis provides regular full time employees with 10 paid holidays annually. Vacation accrual is 17 days per year in the first five years of service and 22 days annually thereafter. Employees do not accrue sick time but take time on an as needed basis.

Leaves — We offer paid leave for jury duty and when bereavement time is needed. Employees may also request a variety of unpaid personal and medical leaves.

Educational Assistance Program — In support of the professional growth and advancement of our employees, we provide financial reimbursement for approved continuing education that relates to current or future positions with the company.

Health Club Membership — You care about your health and so do we! Sunesis offers up to $500 per year for gym membership reimbursement.
